Bond Clinic has been the ground-breaking team in regards to Chronic Care Management in Polk County, FL. We have dedicated an entire department, whose sole focus is working with Chronic Care Patients to help them achieve the health goals set by them and their doctor.

What is Chronic Care Management?
Bond Clinic Quality Management Team works with you and your doctors to help met your health goals. Creation of a personalized care plan and oversight of at least 20 minutes per month via telephone. This program does not replace your visits to the doctor it only enhances communication.
What is the Chronic Care Management Program?
It is a new program by Medicare, designed to keep you out of the hospital when possible. The program allows us time to take better care of you.
How does it work?
Our team will contact you by telephone to review a series of questions. This will help keep you on track to meet your health goals set by you and your doctors. The call is documented in your medical chart for your Primary Care Physician to review.
Who determines if I am eligible for the Chronic Care Management Program?
Your physician will make the final decision if you qualify for the new program. Your physician will speak with you about the program during your clinic visit and have you sign a consent form. You must have 2 or more of the Chronic Diagnosis’s listed in the chart below.
Do I still see my doctor?
Yes. This does not replace regular doctor visits. However, this program may even decrease the frequency of your regular visits if that is what you would like.
How does it work?
It’s a VIP check-in service for patients with chronic disease. A Nurse from our team will phone you every month to check on your progress, and report the results back to your doctor. Based on this information your doctor can fine tune your treatment, guiding you to better health.
What’s the benefit?
In the program, together we will be working closely with your doctor. You will also have 24 hour access to your health care team. Studies show similar programs can reduce unpleasant activities like going to the emergency room and getting admitted to the hospital.
I’m seeing more than one Bond Clinic doctor, do all of them offer this service?
Only one primary care provider can bill for this service. If you see physicians at Bond Clinic we can review your records. You can cancel the service at any time, but we hope you will take advantage of this benefit that allows us to take better care of you.
